How to perform it
Standing at a high pulley with an underhand grip on a straight bar, push the bar down until the elbows are fully straight, then let it rise under control.
- Set a pulley at roughly head height and attach a straight bar.
- Take an underhand grip at about shoulder width.
- Step back half a pace and stand tall.
- Tuck the elbows against your ribs.
- Brace the abs and keep the torso upright.
- Set the wrists straight rather than bent back.
- Push the bar down until the elbows lock out.
- Keep the upper arms still throughout.
- Squeeze for a beat at the bottom.
- Let the bar rise over three seconds until the forearms pass horizontal.

Common mistakes
Elbows drifting forward
If the upper arms travel, the shoulders and lats take over and the triceps do less. Pin the elbows to your sides.
Loading it like an overhand pushdown
The underhand grip is weaker at the wrist, so the usable weight drops. Chasing the old number means bending the wrists back.
Leaning over the bar
Using bodyweight to help the last reps converts an isolation exercise into a poor pressing motion.
Short range at the top
Stopping before the forearms pass horizontal removes most of the lengthened portion.
Expecting it to build a different muscle
It is still a pushdown. The grip changes wrist comfort far more than it changes which head grows.

What the reverse grip pushdown actually trains
A reverse grip pushdown is a cable elbow extension exercise performed standing at a high pulley with the palms facing upwards, pushing a bar down until the arms are straight.
The triceps brachii extends the elbow. Its lateral and medial heads cross the elbow only. Its long head also crosses the shoulder, so with your upper arms by your sides, as they are here, the long head is working in a relatively shortened position.
The claim attached to this exercise is that flipping the grip shifts the emphasis onto the medial or long head. Treat that carefully. Supination happens at the forearm, below the elbow, and the triceps inserts above it. The rotation changes how your wrist and forearm sit, and it changes how much weight you can hold, but the elbow extension itself is not reorganised by turning your hands over.
Kholinne and colleagues found the three heads do contribute differently to elbow extension depending on the demand, so head specific differences are real in principle. Vigotsky and colleagues, though, cautioned that surface electromyography amplitude cannot be read directly as a measure of growth, and most of the popular claims about this exercise trace back to exactly that kind of reading.
What the grip does change is real enough to matter. Lehman found that forearm pronation and supination alter upper body muscle activity during pressing, so grip is not a neutral variable. And a supinated grip puts the wrist in a position many people find more comfortable on a straight bar than the overhand version does.
That is the honest summary. It is a pushdown that some wrists prefer, at a lighter load.
Setting up and executing
Set a pulley at roughly head height and attach a straight bar.
Take an underhand grip at about shoulder width and step back half a pace, standing tall with the torso upright.
Tuck the elbows against your ribs and brace the abs. Set your wrists straight rather than letting them bend backwards under the load.
Push the bar down until the elbows are completely locked out, keeping the upper arms still throughout, and squeeze for a beat at the bottom.
Let the bar rise over about three seconds until your forearms pass the horizontal, which is as much range as this position offers, then push again without letting the stack settle.
The mistakes that cost you the most
Elbows drifting forward is the fault that quietly converts a pushdown into a poor pulldown. Once the upper arms travel, the lats and shoulders start contributing and the triceps does proportionally less. The elbows should be against your ribs on the last rep exactly as they were on the first.
Loading it like an overhand pushdown is the specific error this variation invites. The supinated grip is weaker at the wrist, so the number on the stack has to come down. If it does not, your wrists bend back to cope and the discomfort you were trying to avoid arrives anyway.
Leaning over the bar is the whole body version of the same impatience. If your chest is dropping towards the bar on the last three reps, the set finished earlier than you did.
A short range at the top removes most of the lengthened portion. Pallares and colleagues found full range training beat partial range work, so let the forearms come up past horizontal.
And expecting it to build a different muscle sets you up for disappointment. It is a pushdown with a different grip.
Sets, reps and rest by goal
Moderate to high reps and short rest. This is accessory work and should be treated as such.
For muscle gain, three sets of twelve to fifteen with a minute. For strength, three sets of ten to twelve with ninety seconds. In a fat loss phase, three sets of fifteen to twenty with forty five seconds.
Slow the return to about three seconds. The cable keeps tension on the way up, which is one of its advantages over a dumbbell, and rushing that phase wastes it.
Progress by reps before load. Stack increments on a cable tower are often large relative to what you can push down with a supinated grip, so expect to add reps for a few weeks before the pin moves.
Schoenfeld and colleagues found a dose response between weekly hard sets and hypertrophy, and pushdown variations are a cheap way to add triceps sets that do not tax anything else.
Refalo and colleagues found stopping a rep or two short of failure produces comparable growth to going all the way, though pushdowns carry no risk at failure, so the last set can go the distance.
Where it fits in your split
At the end of a pressing or arm session, after the heavier work.
On push, pull and legs it goes late on push day. On a bro split it follows the overhead triceps work on arm day. On an upper and lower split it is a finisher in either upper session. On a full body programme it is optional and comes last.
The programming point that matters most is ordering. Maeo and colleagues found elbow extension trained with the arm overhead produced substantially greater triceps growth than the same work with the arm neutral. Pushdowns, including this one, train the long head in its short position. So the overhead extension should come first in your triceps work, and pushdowns should fill in behind it.
If your entire triceps programme is pushdowns of various grips, changing the grips will not fix it. Adding an overhead movement will.
Twice a week is reasonable. The triceps already get substantial work from every press you do, so two or three direct sets per session is usually enough.
Variations worth knowing
The overhand pushdown is the standard version and allows more load for what is effectively the same movement, which makes it the better default unless your wrists disagree.
The rope pushdown lets the hands separate and turn at lockout, which most people find the most comfortable option of the three.
A single arm reverse pushdown with a D handle lets the wrist find its own angle and is often kinder still.
The overhead cable extension trains the same muscle in its lengthened position and is the more valuable exercise if you only have room for one.
The cable kickback is another shortened position variant, useful only as a light finisher.
And the machine triceps extension gives you a seat and a fixed path, which makes taking a set to genuine failure straightforward.
What the evidence says
Kholinne and colleagues examined the roles of the three heads of the triceps in elbow extension and found each contributes differently depending on the demand, which is why head specific arguments are worth having at all.
Maeo and colleagues found substantially greater triceps hypertrophy after elbow extension training performed with the arm overhead compared with a neutral arm position, which is the single most important finding for how you order your triceps work and why pushdowns should not be all of it.
Lehman found that grip width and forearm pronation or supination change upper body muscle activity during pressing, which supports the idea that turning the hands over is not a neutral change even if it is a smaller one than commonly claimed.
Vigotsky and colleagues cautioned that surface electromyography amplitude does not translate directly into hypertrophy, which is the correct response to claims that this grip specifically builds the medial head.
Pallares and colleagues found full range training beat partial range training, supporting letting the bar rise past horizontal on every rep.
Schoenfeld and colleagues found a dose response between weekly hard sets and hypertrophy, and Refalo and colleagues found sets stopped short of failure grow muscle comparably to sets taken to it.
Common questions
Does the underhand grip target the medial head?
Not in the way the claim implies. The rotation happens at the forearm, below the elbow the triceps acts on. What changes reliably is wrist comfort and the load you can handle. Kholinne and colleagues showed the heads do behave differently, but grip rotation is not the lever that changes which one grows.
Should I use this instead of a normal pushdown?
Only if your wrists prefer it. The overhand and rope versions allow more load for the same movement, so they are the better default. There is no meaningful growth advantage either way.
Why can I use so much less weight?
Because a supinated grip on a straight bar is mechanically weaker at the wrist and the forearm has to work harder to hold position. That is a grip limitation rather than a triceps one.
Is this a near-duplicate of the overhand pushdown?
Yes, and it is worth saying plainly. Same joint, same arm position, same resistance direction, different hand rotation. Do one of them, not both in the same session.
What should I add if pushdowns are all I do?
An overhead extension, with a cable, a dumbbell or a band. Maeo and colleagues found the overhead arm position produced substantially more triceps growth than the neutral one, and it is the position that pushdowns of every grip leave untrained.
