How to perform it

Facing away from a high cable with a rope held behind your head, extend the elbows to drive your hands forwards and up until the arms are straight.

  1. Attach a rope to a high cable pulley.
  2. Take an end in each hand and turn to face away from the machine.
  3. Bring the rope over your shoulders so your hands sit behind your head.
  4. Step forward into a split stance until the cable is under tension.
  5. Hinge slightly forward at the hips and brace the abs.
  6. Set the elbows close to your head and pointing forwards.
  7. Extend the elbows to drive the hands forwards and upwards.
  8. Separate the rope ends slightly at lockout.
  9. Pause for a beat with the arms straight.
  10. Bend the elbows over three seconds until you feel a deep stretch.

Common mistakes

Elbows flaring wide

Letting the elbows spread turns this into a pressing motion and reduces the load on the triceps.

Standing too upright

Without a slight forward lean the cable pulls you backwards and you spend the set fighting for balance.

Short range at the back

Not letting the elbows bend fully removes the stretched position that makes this exercise worth doing.

Going too heavy

Excess load pulls the elbows apart and turns the movement into an awkward overhead press.

Feet together

A square stance leaves you unstable. A split stance gives you something to brace against.

What the rope overhead triceps extension actually trains

A rope overhead triceps extension is an elbow extension exercise performed facing away from a high cable, with a rope held behind your head, driving the hands forwards and upwards until the arms are straight.

All three heads of the triceps brachii extend the elbow, but only the long head crosses the shoulder as well. That single anatomical fact is what makes this exercise worth its awkward setup. With the arms overhead, the shoulder is flexed, the long head is placed under stretch, and it stays loaded through a range that no pushdown can reach.

Kholinne and colleagues examined the roles of the three heads and found they contribute differently to elbow extension, which is why treating the triceps as one undifferentiated muscle leads to lopsided training.

Maeo and colleagues supplied the strongest practical finding in triceps training. Elbow extension performed with the arms overhead produced substantially greater triceps hypertrophy, particularly in the long head, than identical work performed with the arms at the sides. The long head is the largest of the three, so this is not a minor detail.

The rope adds two useful things. The hands can separate at lockout, which lets the wrists sit comfortably and gives a stronger finishing contraction. And a cable, unlike a dumbbell or bar overhead, maintains tension at the top rather than letting the load rest over the joint.

If you only have room for one triceps isolation exercise in your programme, this is the one to keep. Pushdowns are more comfortable and easier to set up, which is why they get chosen, but they leave the biggest head of the muscle undertrained.

Setting up and executing

Attach a rope to a high pulley. Take an end in each hand and turn to face away from the machine, bringing the rope over your shoulders so your hands sit behind your head.

Step forward into a split stance until the cable is under tension. A square stance leaves you fighting for balance; a split stance gives you something to brace against.

Hinge slightly forward at the hips and brace the abs. The lean is what stops the cable pulling you over backwards.

Set the elbows close to your head and pointing forwards. They stay there.

Extend the elbows to drive the hands forwards and upwards, separating the rope ends slightly at lockout, and pause for a beat.

Bend the elbows over three seconds until you feel a deep stretch along the back of the upper arm, then extend again.

The mistakes that cost you the most

Elbows flaring wide is the fault that turns this into an overhead press. When the elbows spread, the shoulders and chest contribute and the triceps unloads. Keep them close to your head and pointing forwards, even if that costs you weight.

Standing too upright is the setup version of the same problem. Without a forward lean and a split stance, the cable pulls you backwards and most of your effort goes into staying upright rather than extending your elbows.

Short range at the back is the most expensive error, because the stretched position is the entire reason this exercise exists. Schoenfeld and Grgic reviewed range of motion for muscle development and concluded that fuller ranges are generally better, with the lengthened portion carrying particular weight.

Going too heavy causes all of the above at once. The honest load is lighter than your pushdown, sometimes considerably. That is not a flaw; the long head is being asked to work in a stretched position where it is weaker.

And feet together leaves you unstable enough that you will shorten the range instinctively to stay balanced. Split the stance.

Sets, reps and rest by goal

This is the triceps movement worth giving real sets to.

For muscle gain, four sets of ten to fifteen with ninety seconds rest. For strength, three sets of eight to twelve with two minutes. In a fat loss phase, three sets of fifteen to twenty with a minute.

Take three seconds on the way back and pause briefly in the stretch before extending. Pedrosa and colleagues found partial repetitions at long muscle lengths produced favourable hypertrophy compared with short length partials, which means the bottom of this movement is the part to protect when fatigue arrives.

Progress in small increments. A cable stack usually moves in five kilo steps, which is a large jump on an overhead extension, so add reps within a weight until you reach the top of the range on every set.

Schoenfeld and colleagues found a dose response between weekly hard sets and hypertrophy. Six to twelve direct triceps sets a week is a reasonable target, and it is worth weighting those towards overhead work rather than pushdowns.

Refalo and colleagues found sets stopped a rep or two short of failure produce comparable growth to sets taken to failure. Here the natural stopping point is the rep where your elbows start to flare, which usually arrives first.

Where it fits in your split

After the pressing, as the main triceps movement of the session.

On push, pull and legs it goes on push day after the bench and overhead work. On a bro split it is the first triceps exercise on arm day. On an upper and lower split it follows the pressing in either upper session. On a full body programme it is worth a slot whenever a cable station is free.

Do it before pushdowns. The stretched overhead position is the part of triceps training that is easy to skip and impossible to replace, so spend your freshest sets on it and finish with something easier.

Two sessions a week works well, and much of your triceps volume already comes from pressing.

One caution. Overhead work asks for a certain amount of shoulder flexion, and if you cannot get your arms comfortably overhead you will compensate by arching your lower back or flaring your elbows. Kolber and colleagues reviewed shoulder injuries attributed to resistance training and identified overhead positions as worth treating carefully. If the position is uncomfortable, use a single arm version or a lying extension instead, which achieves much of the same stretch with less shoulder demand.

Variations worth knowing

The single arm overhead cable extension works one side at a time, which exposes differences between arms and lets each shoulder find its own comfortable angle.

The seated overhead dumbbell extension holds one dumbbell in both hands behind the head, and is the best version available without a cable station.

The EZ bar overhead extension loads more easily and progresses in smaller steps, but loses tension at lockout where the bar sits over the joint.

The lying triceps extension on a bench angles the arms back rather than fully overhead, which is a partial version of the same position and easier to set up.

The cable pushdown works the arms at the sides and trains the lateral and medial heads, which makes it a sensible partner rather than a rival.

And the overhead band extension covers the same movement pattern when no cable is available, with the caveat that band resistance is lightest where you want it heaviest.

What the evidence says

Maeo and colleagues found that triceps hypertrophy was substantially greater after elbow extension training performed in the overhead position than in a neutral arm position, with the long head showing the largest difference. This is the clearest evidence in triceps training and it is the reason this exercise belongs in your programme.

Kholinne and colleagues examined the roles of the long, lateral and medial heads during elbow extension and found they are not interchangeable, which supports pairing overhead work with a pushdown rather than choosing one and ignoring the other.

Pedrosa and colleagues found partial repetitions performed at long muscle lengths produced favourable hypertrophy compared with short length partials, which is why the deep stretch at the back of this movement matters more than the lockout.

Schoenfeld and Grgic reviewed range of motion and muscle development and concluded that fuller ranges generally produce better hypertrophy, supporting letting the elbows bend completely.

Schoenfeld and colleagues found a dose response between weekly hard sets and hypertrophy, which sets the weekly target once pressing volume is counted.

Refalo and colleagues found comparable hypertrophy from sets stopped short of failure, which suits a movement where technique degrades before the muscle does.

Kolber and colleagues reviewed shoulder injuries attributed to resistance training and flagged overhead positions as worth approaching with care, which is the reason to check your shoulder mobility before loading this heavily.

Common questions

Why is this better than a pushdown?

Because the long head of the triceps crosses the shoulder, so it is only stretched when your arms are overhead. Maeo and colleagues found substantially greater triceps growth, especially in the long head, from overhead training than from the same work with the arms at the sides. A pushdown trains the other two heads well and the largest one poorly.

High pulley or low pulley?

A high pulley behind you gives you tension at the stretched position and at lockout, which is what you want. A low pulley with the arms overhead also works and some people find it easier to set up. Both beat a dumbbell, which has no tension at the top.

Why do I keep falling forwards or backwards?

Because your stance is square. Step into a split stance and lean slightly forward from the hips so the cable has something to pull against. Balance should not be part of this exercise.

My shoulders will not go overhead comfortably. What now?

Use a single arm version, which lets you angle the arm where it is comfortable, or switch to a lying extension on a bench, which angles the arms back without asking for full overhead flexion. Reviews of resistance training shoulder complaints flag overhead positions as worth treating carefully, so forcing it is not the answer.

Is this different enough from a lying triceps extension to do both?

They overlap heavily, since both put the upper arms behind the torso and load the long head. The overhead version does it more completely and keeps tension at lockout. If you have one slot, take this. If you have two, do this early and the lying version on your second session for variety, not because they train different things.
