How to perform it

Lying face down on the machine's pad with the hips at the edge, swing both legs back and up into full hip extension, then lower them under control.

  1. Set the load light for the first set, since leverage on these machines varies.
  2. Lie face down with the hips just past the edge of the pad.
  3. Hook the feet or ankles into the strap or roller as the design requires.
  4. Grip the handles firmly and pull the chest into the pad.
  5. Let the legs hang down and relax into the bottom position.
  6. Brace the abs so the lower back does not lead the movement.
  7. Swing the legs back and up by squeezing the glutes.
  8. Stop when the hips are straight and the pelvis stops rotating.
  9. Hold the top position for a beat without arching.
  10. Lower over two to three seconds and repeat without bouncing.

Common mistakes

Swinging with momentum

Using a big pendulum swing means the machine does the work at both ends of the range.

Arching the lower back at the top

Once the hips are straight, extra height comes from the spine rather than the glutes.

Letting the legs drop

The lowering is where the hamstrings get loaded. Control it for two to three seconds.

Hips positioned wrong on the pad

Too far forward and the movement happens at the spine, too far back and the range shrinks.

Chasing the decompression story

Use it because it trains hip extension well, not because you have been promised it will fix your back.

What the reverse hyperextension actually trains

A reverse hyperextension is a hip extension exercise performed lying face down on a machine with the torso fixed on a pad, in which you swing both legs back and up against resistance rather than lifting the torso.

The gluteus maximus does most of the work, with the hamstring group, principally biceps femoris and semitendinosus, assisting throughout. The erector spinae works to keep the spine steady, and the abs brace to stop the pelvis leading the movement.

The distinguishing feature is which end moves. In a back extension, the legs are anchored and the torso rises. Here the torso is anchored and the legs rise. The muscular action at the hip is the same, but the spine is doing far less, and the load on the lumbar region is much lower than in a loaded hinge.

That combination is genuinely useful. It gives you hip extension volume with modest spinal cost, which makes it a good pairing with heavy squatting and deadlifting rather than a competitor to them.

Now the part that needs saying plainly.

This machine arrived in gyms with a large claim attached: that swinging the legs creates a traction and pumping effect that decompresses and rehabilitates the lower back. That story is repeated everywhere and it is not well established. It may be true, it may be partly true, and there is not the evidence to say so with the confidence the claim is usually made with.

Cholewicki and colleagues measured lumbar spine loads during very heavy lifting, and McGill has written extensively on training the trunk for performance and injury prevention. Neither of those bodies of work amounts to an endorsement of the decompression story. Use the machine because it trains hip extension well with a light spine, which is reason enough.

Setting up and executing

Start light. Leverage differs enormously between reverse hyper designs and a load that felt trivial on one machine can be unmanageable on another.

Lie face down with your hips just past the edge of the pad. Too far forward and the movement happens at the spine; too far back and the range shrinks to nothing.

Hook the feet or ankles into the strap or roller, depending on the design.

Grip the handles firmly and pull your chest into the pad. Let the legs hang down and relax into the bottom position.

Brace the abs so the lower back does not lead the movement.

Swing the legs back and up by squeezing the glutes, stopping when the hips are straight and the pelvis stops rotating. Hold for a beat, then lower over two to three seconds without bouncing at the bottom.

The mistakes that cost you the most

Swinging with momentum is the fault the machine invites, because the legs hang like a pendulum and a pendulum wants to keep going. If you let it, the bottom of each rep is a bounce and the top is a follow through, and the glutes contribute in a narrow window in between. Slow the whole thing down and the honest load drops dramatically, which tells you what was happening.

Arching the lower back at the top is the same error as on every hip extension exercise. Once the hips are straight there is no more hip extension available, and any additional height comes from the lumbar spine. That is not extra glute work and it is the one thing you were supposed to be avoiding here.

Letting the legs drop wastes the lowering. Maeo and colleagues found greater hamstring hypertrophy from training at long muscle lengths, and the bottom of this movement is where the hamstrings are lengthened under load. Falling through it gives that away.

Hips positioned wrong on the pad is a setup error that changes which joint moves. Check it before you load.

And chasing the decompression story leads people to do endless light swinging sets in the hope of a therapeutic effect, rather than a few controlled sets that actually train something.

Sets, reps and rest by goal

Moderate to high reps, controlled tempo, short rests.

For muscle gain, three sets of twelve to twenty with a minute of rest. For strength, three sets of ten to fifteen with ninety seconds. In a fat loss phase, three sets of fifteen to twenty five with forty five seconds.

Take the lowering slowly and pause briefly at the bottom to kill the pendulum. A set of twelve strict reps is far harder and far more useful than twenty swung ones.

Schoenfeld and colleagues found a dose response between weekly hard sets and muscle growth, and this is a convenient way to add posterior chain volume on a day when your spine has already had enough from squatting and deadlifting.

Refalo and colleagues found stopping a rep or two short of failure produces comparable growth to going all the way, which fits here because the reps near failure are the ones that turn into swinging.

Where it fits in your split

Late in a lower body session, after the compound work.

On push, pull and legs it goes on legs day after the squatting and hinging. On an upper and lower split it fits either lower session towards the end. On a bro split it belongs on leg day, or on back day if that is where your hinging lives. On a full body programme it is optional and depends on the machine being free. On a bodyweight programme it is replaced by glute bridges or a bench version.

The strongest case for including it is on a day where your lower back has already taken a lot. After heavy deadlifts, most posterior chain accessories add more spinal fatigue on top. This one adds hip extension work with far less of that, which is a genuinely useful property.

The second case is variety at the bottom of the range. The hanging start position loads the hips in a way that a hip thrust or a back extension does not.

Variations worth knowing

The bench reverse hyper, lying face down on a flat bench and raising the legs with bodyweight or a light dumbbell held between the feet, is the version available in any gym and behaves similarly at low loads.

The back extension flips the anchor, fixing the legs and moving the torso, which trains the erectors much more directly.

The cable pull-through trains the same hip extension action from standing, with a longer range and a hamstring stretch this machine does not offer.

The hip thrust is the heavy option, loading hip extension with the shoulders supported and allowing far more weight than anything here.

The single leg version, swinging one leg at a time, exposes side to side differences and halves the load requirement.

And the glute bridge on the floor is the equipment free equivalent, useful at home and easy to progress to a single leg version.

What the evidence says

Cholewicki and colleagues measured lumbar spine loads during the lifting of extremely heavy weights, which provides the context for why an exercise that trains the hips without loading the spine heavily is worth having in a programme built around squats and deadlifts.

McGill wrote on core training and its translation to performance and injury prevention, which supports bracing the trunk and keeping the spine still rather than treating this as a lumbar exercise.

Bourne and colleagues examined how exercise selection affects hamstring activation and found substantial differences between movements, which is relevant because hip extension exercises share the work between glutes and hamstrings in patterns that shift with knee and hip position.

Maeo and colleagues found greater hamstring hypertrophy after training at long muscle lengths, supporting a controlled lowering into the hanging bottom position rather than a bounce.

Vigotsky and colleagues wrote about interpreting surface electromyography amplitudes, which is worth remembering whenever someone ranks posterior chain exercises by measured activation and presents the ranking as a training recommendation.

Schoenfeld and colleagues on weekly volume and Refalo and colleagues on proximity to failure supply the programming, and both argue for a small number of controlled sets rather than long swinging ones.

Common questions

Does this decompress my spine?

The claim is popular and the supporting evidence is much weaker than the confidence with which it is stated. There is nothing wrong with the exercise, and it may well feel good on your back, but treat the therapeutic story as unproven and choose the movement for the hip extension training it provides.

How is this different from a back extension?

Which end moves. A back extension anchors the legs and lifts the torso, which loads the spinal erectors heavily. A reverse hyper anchors the torso and lifts the legs, which keeps the emphasis at the hip and the spinal demand low. They are complements, not duplicates.

Why does my lower back arch at the top?

Because your hips have run out of extension and your body is finding more range at the spine. Stop the legs when the hips are straight. The finishing position is lower than most people expect and the set gets harder, not easier, when you respect it.

How heavy should I go?

Heavy enough that twelve to twenty controlled reps are difficult, and no heavier. Once you need a swing to move the weight, the machine's pendulum is doing the work and the exercise has stopped being useful.

Can I do this without the machine?

Yes. Lie face down on a flat bench with your hips at the edge, hold the bench, and raise your legs. It is a reasonable substitute at bodyweight, though adding load means squeezing a dumbbell between your feet, which is awkward past a certain point.
