Overhead Triceps Extension
The one triceps exercise that trains the long head at full length, which is why it is worth more than a fourth variation of pushdown.

How to perform it
With the upper arms alongside the head, lower the weight behind you and extend the elbows back to lockout.
- Set a cable pulley low and attach a rope, or take a single dumbbell in both hands.
- Face away from the stack, step forward into a split stance and bring the rope overhead behind you.
- Raise the upper arms so they sit alongside your head, elbows pointing forward.
- Brace the abs and set the ribs down so the lower back does not arch as the load pulls back.
- Lower the weight behind your head by bending the elbows only.
- Keep the upper arms exactly where they are. If they drop, the set has changed exercise.
- Continue until the forearms pass parallel with the floor and you feel a stretch along the back of the arm.
- Extend the elbows and drive the hands forward and up to full lockout.
- Spread the rope apart slightly at the end if you are using one, and pause for a beat.
- Lower over about two seconds and repeat without resting at the top.

Common mistakes
Letting the elbows drop
As the set gets hard the upper arms start falling forward, which shortens the lever and hands the work to the shoulders. The upper arms sit beside the head and stay there. When they cannot, the set is over.
Arching the lower back
The overhead position pulls the ribs upward, and the easy compensation is to arch. Brace the abs, set the ribs down, and use a split stance so the pelvis has something to work against.
Cutting the stretch short
Stopping at ninety degrees skips the deep position that makes this exercise different from a pushdown. Let the forearms pass parallel every rep.
Elbows flaring wide
Letting the elbows spread out to the sides reduces the load on the triceps and adds joint strain. Keep them pointing forward and roughly shoulder width.
Loading it like a pressdown
The overhead position is mechanically weaker and the stretched position is demanding. Expect to use less weight than you would on a pushdown, and be fine with that.

Variations and swaps
Cable rope overhead extension
Constant tension through the whole range, including lockout. The version most worth doing if a cable stack is free.
Single dumbbell overhead extension
One dumbbell held in both hands behind the head. The simplest version and the one that needs no equipment beyond a dumbbell.
Single-arm overhead extension
One arm at a time with a dumbbell or cable. Exposes side to side differences and lets the elbow find its own path.
Seated overhead extension
Sitting with back support removes the lower back arching entirely. The fix if your ribs flare no matter how hard you brace.
EZ bar overhead extension
The angled grip suits the wrists better than a straight bar. A good middle ground between a rope and a dumbbell.
Skull crusher
The lying cousin. Less shoulder flexion, so slightly less long head stretch, but easier to load heavily.
What the overhead triceps extension actually trains
The triceps has three heads and only one of them crosses the shoulder. The long head runs from the shoulder blade to the forearm, which means its length depends on where your upper arm is as well as how bent your elbow is. Raise the arm overhead and the long head is stretched. Keep it by your side and it is not.
That single anatomical fact is why this exercise exists as something more than a pushdown done awkwardly.
Maeo and colleagues trained one group with the shoulder flexed, arms overhead, and another with the arms by the side, using the same elbow extension movement. Long head hypertrophy was substantially greater in the overhead group. The long head is the largest of the three heads, so the difference is not cosmetic.
Kholinne and colleagues examined the distinct contributions of each head during elbow extension and found the heads behave differently depending on position and demand, which supports the general idea that arm position is a training variable rather than a comfort preference.
The practical conclusion is simple. If your triceps work consists entirely of pressing and pushdowns, every rep you have ever done has trained the long head short. This is the exercise that fixes that.
Setting up and executing
The cable version is the best if a stack is free. Set a low pulley, attach a rope, face away from the machine and step forward into a split stance. Bring the rope overhead behind you and raise the upper arms so they sit alongside your head with the elbows pointing forward.
Brace the abs and set the ribs down. The overhead position pulls the ribcage upward, and the standard compensation is to arch the lower back. The split stance and a firm brace prevent it.
Lower the weight behind your head by bending the elbows only. The upper arms stay exactly where you put them. Continue until the forearms pass parallel with the floor and you feel a clear stretch down the back of the arm.
Then extend the elbows and drive the hands forward and up to full lockout. If you are using a rope, spread the ends apart slightly at the end. Pause for a beat, lower over about two seconds, and go again without resting at the top.
The mistakes that cost you the most
The elbows dropping is the fault that ends most sets before the triceps are actually done. As fatigue builds, the upper arms start falling forward towards vertical, which shortens the lever and hands part of the job to the shoulders. It feels like you are still doing the exercise. You are not. When the upper arms will not stay beside your head, the set is finished.
Arching the lower back is the second, and it is a direct consequence of the overhead position. Brace hard, set the ribs down, use a split stance. If it still happens, sit down with back support.
Cutting the stretch short is the one that quietly removes the entire point. Stopping at ninety degrees means you have done a pushdown with a strange setup. Pallares and colleagues found full range training generally outperformed partial range training for adaptations, and here the deep portion is exactly the part that distinguishes this exercise from everything else in your programme.
Flaring the elbows out sideways reduces triceps load and adds joint strain. Keep them forward.
And loading it like a pushdown is the last one. The overhead position is mechanically weaker and the stretched position is genuinely demanding. Less weight is correct, not a failure.
Sets, reps and rest by goal
Moderate to high reps work best. The stretched position is already demanding, and very heavy loading here tends to produce elbow discomfort rather than progress.
For muscle gain, three to four sets of ten to fifteen with about seventy five seconds of rest. For strength work, three sets of eight to twelve with ninety seconds, though if pressing strength is the goal, close grip bench work is the better tool. In a fat loss phase, three sets of twelve to twenty with a minute of rest.
Schoenfeld and colleagues found a dose response between weekly hard sets and hypertrophy, and it is worth remembering that pressing already contributes substantial triceps volume. Two dedicated triceps exercises a week is enough for most people. Refalo and colleagues found that sets stopped a couple of reps short of failure produce comparable hypertrophy to going all the way, which suits an exercise where the last rep is usually the one where the elbows collapse forward.
Where it fits in your split
After pressing, always. Fatiguing the triceps before a bench press or overhead press costs more on the compound than it gains on the isolation.
On push, pull and legs it goes on push day. On an upper and lower split it sits in the accessory block. On a bro split it belongs on arm day, and the sensible pairing is one stretched-position movement like this and one shortened-position movement like a pushdown, so the muscle gets trained at both ends of its range.
That pairing is the single most useful piece of triceps programming there is, and it takes about eight minutes a week.
Variations worth knowing
The cable rope version is the best if you have access to a stack. A cable keeps tension on the muscle at lockout, where a dumbbell gives you almost none because the load line runs straight down through the elbow.
A single dumbbell held in both hands behind the head is the simplest version and needs nothing else. It is a perfectly good exercise, just with a resistance curve that fades at the top.
Single-arm work with a dumbbell or cable lets each elbow find its own path and exposes side to side differences. Useful if one arm consistently gives out first.
Seated with back support removes the lower back arching entirely. If your ribs flare no matter how hard you brace, stop fighting it and sit down.
An EZ bar is a good middle ground, more comfortable on the wrists than a straight bar and easier to load than a rope.
And the skull crusher is the lying relative. Less shoulder flexion, so slightly less long head stretch, but easier to load heavily. Running both across a block is more sensible than picking a winner.
What the evidence says
The Maeo study is the one that matters. Training elbow extension with the shoulder flexed produced substantially greater long head hypertrophy than training with the arm by the side, measured by muscle size after a training period rather than by activation during a session. That is a stronger form of evidence than most exercise comparisons offer.
Kholinne and colleagues support the underlying principle by showing the three heads contribute differently depending on position and demand.
Pallares and colleagues found full range training generally beat partial range training, which is the argument for letting the forearms pass parallel rather than stopping at a comfortable ninety degrees.
Schoenfeld and colleagues and Roig and colleagues both found modest advantages for eccentric-emphasised training, which is the case for a controlled two second lowering rather than letting the weight fall behind your head.
Vigotsky and colleagues provide the standard caution about surface electromyography. Rankings of triceps exercises by activation amplitude are common and do not establish which builds more muscle. The Maeo result is worth more than all of them because it measured the outcome people actually care about.
Common questions
Why is the overhead position better for the long head?
Because the long head crosses the shoulder joint. When your arm is overhead, that head is already stretched before you even bend the elbow, and it stays under tension at a long muscle length throughout the rep. When your arm is by your side, as in a pushdown, it is slack. Training a muscle at long lengths tends to produce more growth, and the Maeo study found exactly that for this muscle.
Cable or dumbbell?
Cable if one is free, because the tension stays on through lockout rather than disappearing at the top. A dumbbell is a perfectly good substitute and needs no equipment beyond itself. The difference is real but small.
My lower back arches. How do I stop it?
Use a split stance rather than a square one, brace the abs before the first rep, and consciously pull the ribs down. If it still happens, do the exercise seated with a back pad. That removes the problem entirely rather than managing it.
Do I still need pushdowns if I do these?
They are worth keeping. This movement trains the triceps at long muscle lengths, a pushdown trains it in a shortened position where the lateral and medial heads contribute strongly. One of each covers the muscle properly, and the whole pairing takes about eight minutes.
How heavy should I go?
Lighter than your pushdown. The stretched position is demanding and the overhead position is mechanically weaker, so the load will be noticeably lower. Judge the set by whether the upper arms stayed beside your head and the forearms passed parallel, not by the number on the stack.
