How to perform it
Lie face down on a reverse hyperextension bench with the legs hanging free, then raise them behind you until the hips are extended.
- Set the pad height so your hips sit at the edge of the bench.
- Lie face down with the legs hanging freely below.
- Grip the handles and pull your chest into the pad.
- Brace the abs so the ribs stay down.
- Let the legs hang straight down to start.
- Squeeze the glutes and raise the legs behind you.
- Keep the knees straight or very slightly bent.
- Stop when the hips are level rather than going higher.
- Pause briefly with the glutes contracted.
- Lower over two to three seconds without letting the legs swing.

Common mistakes
Swinging the legs
Using momentum turns the machine into a pendulum. Control both directions and the muscles have to work.
Going too high at the top
Driving the legs above hip level extends the lumbar spine rather than the hips. Stop when the hips are straight.
Arching instead of squeezing
If the movement comes from the lower back rather than the glutes, the target muscles are being skipped.
Torso lifting off the pad
Losing contact with the bench means the load has outgrown your ability to anchor yourself.
Treating it as spinal therapy
It is a hip extension exercise, not a treatment. Programme it as training, not as a fix for a sore back.

What the reverse hyperextension actually trains
A reverse hyperextension is a hip extension exercise performed face down on a raised bench with the torso fixed and the legs hanging free, raising the legs behind you against a weighted strap, pendulum or pad.
The glutes and hamstrings do the work of extending the hips. The erector spinae contract isometrically to hold the spine in position, and the adductors contribute more than most people expect.
Which brings us to the honest framing. This is not a lower back exercise in the sense people mean when they say it.
The lumbar erectors are working, but they are working the way they work in a plank, which is holding a position rather than producing the movement. The movement itself is hip extension, and hip extension is glutes and hamstrings. If you programme this expecting it to build a thick set of spinal erectors, you have picked the wrong tool; a loaded carry, a heavy hinge or a proper back extension does more.
The second claim attached to this machine is decompression. The idea is that the swinging legs traction the spine and that this is therapeutic. It is a plausible sounding mechanism with limited support. Honestly, it is a hip extension exercise that many people find comfortable on their back, which is a good enough reason to do it without inventing a treatment.
What it genuinely offers is hip extension training with almost no axial loading. Cholewicki and colleagues measured very high compressive loads on the lumbar spine during heavy lifting, and the value of this machine is that it trains the same movement pattern with essentially none of that. That is useful in a week already full of squats and deadlifts.
Setting up and executing
Set the pad height so your hips sit right at the edge of the bench. Too far forward and your lower back does the work; too far back and the range disappears.
Lie face down with the legs hanging freely below you.
Grip the handles and pull your chest firmly into the pad. Brace the abs so the ribs stay down and the lower back does not arch to start the rep.
Let the legs hang straight down. That is the start position, and each rep begins from there rather than from the top of a swing.
Squeeze the glutes and raise the legs behind you, knees straight or very slightly bent, stopping when the hips are level. Higher is not better.
Pause briefly with the glutes contracted, then lower over two to three seconds without letting the legs swing away from you.
The mistakes that cost you the most
Swinging the legs is the fault the machine invites, because the pendulum design makes momentum almost free. If the weight travels because of the previous rep rather than because your glutes moved it, you are exercising the machine. Pause at the bottom and control the descent.
Going too high at the top is the second half of the same problem. Once the hips are straight, further movement comes from extending the lumbar spine, which is precisely what you were told this machine avoids. Stop level.
Arching instead of squeezing means the lower back is initiating the rep. Bourne and colleagues found that hamstring activation varies substantially depending on the exercise and how the hip and knee are positioned, and the practical version of that here is that the glutes and hamstrings only do their job if you make them start the movement.
Torso lifting off the pad means the load has outgrown your anchor. Grip harder, or take weight off.
And treating it as spinal therapy leads people to do it while injured and to expect something it does not deliver. It is training. Programme it as training.
Sets, reps and rest by goal
Moderate to high reps with strict control. This is not a movement to grind.
For muscle gain, three sets of twelve to fifteen with ninety seconds rest. For strength, three sets of eight to twelve with two minutes, though strength here is a secondary concern. In a fat loss phase, three sets of fifteen to twenty with a minute.
Control the eccentric deliberately. Schoenfeld and colleagues compared concentric and eccentric muscle actions and found eccentric work at least as effective for hypertrophy, and on this machine the descent is the part that most people surrender to gravity.
Pallares and colleagues found full range training produced better outcomes than partial range work, which here means a genuine hang at the bottom rather than a shortened swing between two mid range positions.
Schoenfeld and colleagues found a dose response between weekly hard sets and hypertrophy, and if hip extension volume is what you are after, this competes with hinges and thrusts rather than adding to your back work.
Refalo and colleagues found training close to failure produces comparable growth to training at failure, which is a good rule on a machine where the failure rep is where the swinging starts.
Where it fits in your split
Late in a lower body session, after the main hinge work.
On push, pull and legs it goes on legs day after the squats and deadlifts. On a bro split it fits as a finisher on leg day or, if you prefer, at the end of back day. On an upper and lower split it belongs in the lower session, late. On a full body programme it is optional and depends on the machine being free.
The strongest case for it is on a week where your hinge volume is already high and your spine has had enough. A Romanian deadlift and a reverse hyperextension train overlapping muscles, but one of them puts several hundred kilos of compression through your lumbar spine and the other does not. Rotating some volume to the machine in a heavy block is sensible.
The weakest case is programming it as a back exercise and believing it does something rows do not.
Once a week is plenty. Twice if it is replacing hinge volume rather than adding to it.
Variations worth knowing
The back extension, on a forty five degree or horizontal bench, is the mirror image, moving the torso over fixed legs, and is easier to find in most gyms.
The bodyweight reverse hyper, done lying on a flat bench or a plyo box with your hips at the edge, requires no machine at all and is a perfectly reasonable substitute.
The banded reverse hyper adds resistance that increases towards the top of the range, which suits the strength curve of hip extension.
The Romanian deadlift is the loaded hinge alternative, offering far more total stimulus for the glutes and hamstrings at the cost of substantial spinal loading.
The hip thrust is a heavier glute focused hip extension with a much shorter range and no spinal loading to speak of.
And the Nordic curl trains the hamstrings through knee flexion instead, which is the function this movement leaves out.
What the evidence says
Bourne and colleagues compared hamstring activation across exercises and found substantial differences depending on the movement and joint positions involved, which supports treating this as hip extension training rather than assuming it covers the hamstrings completely.
Cholewicki and colleagues measured lumbar spine loads during heavy lifting and found very high compressive forces, which is the clearest argument for having a hip extension option that avoids axial loading altogether.
McGill argued that trunk training is primarily about stiffness and control rather than producing movement, which describes exactly what the erector spinae are doing here, holding position rather than driving the rep.
Pallares and colleagues found full range training beat partial range training for strength and hypertrophy, supporting a full hang at the bottom of each repetition.
Schoenfeld and colleagues compared concentric and eccentric muscle actions and found eccentric work at least as effective, which is why controlling the descent matters on a machine designed to swing.
Schoenfeld and colleagues also found a dose response between weekly hard sets and hypertrophy, which is how to weigh this against your other hip extension work.
Refalo and colleagues found training close to failure produces comparable hypertrophy to training at failure, supporting stopping when control goes rather than when the legs stop rising.
Common questions
Is this a back exercise or a leg exercise?
A leg exercise. Hip extension is driven by the glutes and hamstrings; the spinal erectors hold position isometrically. It gets filed under back in most gyms because of where the machine sits and what it looks like, not because of what it trains.
Does it really decompress the spine?
That claim is more confident than the evidence supports. What is fair to say is that it trains hip extension with almost no compressive loading, which many people find comfortable. Use it without calling it treatment.
How high should I lift my legs?
Until your hips are straight and no further. Going above that comes from extending the lumbar spine, which removes the main advantage of the machine and reintroduces exactly the loading you were avoiding.
How is it different from a back extension?
A back extension moves the torso over fixed legs; a reverse hyper moves the legs under a fixed torso. Both are hip extension movements training the same muscles, and the practical difference is mostly which one your gym has and which one feels better.
Can I do it without the machine?
Yes. Lie face down on a flat bench or a box with your hips at the edge and raise your legs, adding a light dumbbell between the feet or a band around the ankles once bodyweight becomes easy. It is not as smooth, but it trains the same thing.
