How to perform it

Standing side on to a cable set at elbow height with the elbow tucked to your side, rotate the forearm inwards across your stomach.

  1. Set a cable pulley to roughly elbow height and attach a single handle.
  2. Stand side on with the working arm nearest the machine.
  3. Take the handle with the near hand and step away to create tension.
  4. Tuck the working elbow against your ribs, bent to ninety degrees.
  5. Hold a rolled towel between the elbow and your side if it drifts.
  6. Set the shoulder blade down without shrugging.
  7. Rotate the forearm inwards across your stomach.
  8. Stop when the forearm reaches the midline of your body.
  9. Pause briefly at the end of the range.
  10. Return slowly outwards under control.

Common mistakes

Elbow leaving the ribs

A floating elbow lets the chest and front delt take over. Pin it or use a towel.

Heavy loads turn a small rotation into a whole body pull. Stay light.

Turning the torso

Twisting the body makes the range look bigger while the shoulder does less.

Doing it instead of external rotation

The internal rotators are already trained by all your pressing. External rotation is the neglected side.

This is a small accessory at most. It should never occupy meaningful session time.

What cable shoulder internal rotation actually trains

Cable shoulder internal rotation is an isolation exercise in which you stand side on to a cable with your elbow tucked against your ribs and rotate your forearm inwards across your stomach.

The muscles doing the work are the internal rotators of the shoulder: the subscapularis of the rotator cuff, along with the pectoralis major, latissimus dorsi and teres major, all of which rotate the humerus inwards.

Now the part most articles about this exercise avoid saying. Most lifters do not need it.

The internal rotators are already among the hardest worked muscles in a normal training week. Every bench press, every overhead press, every dip and push-up involves the chest and lat pulling the arm inwards, and the subscapularis works alongside them. Saeterbakken and colleagues found bench press variations produce substantial activity in the muscles that drive the arm across and inwards, which is the pattern you repeat hundreds of times a month without thinking about it.

Its counterpart, external rotation, gets almost nothing. That imbalance is why external rotation is the exercise worth your five minutes and this one usually is not.

There are genuine exceptions. If you are working through a specific shoulder problem where the subscapularis has been identified as weak, if you throw or serve for sport and want the internal rotators trained in the overhead position specifically, or if you have been prescribed it as part of a rehabilitation plan, then it has a clear job. Outside those cases, it is filler.

Kolber and colleagues reviewed shoulder injuries attributed to resistance training and found the shoulder to be commonly affected in lifters, but the practical takeaway from that literature points towards balancing pressing with pulling and external rotation, not towards adding more internal rotation work.

Setting up and executing

Set a cable pulley to roughly elbow height and attach a single handle.

Stand side on to the machine with your working arm nearest to it. Take the handle with that hand and step away until there is tension at the start.

Tuck the working elbow against your ribs, bent to ninety degrees. Roll a towel and hold it between your elbow and your side if the elbow drifts.

Set the shoulder blade down without shrugging.

Rotate the forearm inwards across your stomach, stopping when it reaches the midline. Pause briefly.

Return slowly outwards under control, keeping the elbow pinned, and repeat.

The mistakes that cost you the most

The elbow leaving the ribs is the fault that changes the exercise. Once the upper arm floats away, the chest and front deltoid take over and you are doing a poor cable fly rather than a rotation. Pin the elbow and check it.

Too much weight causes that and everything else. The subscapularis is small and the honest load is very light. If the movement requires a lean or a swing, it has already stopped working.

Turning the torso makes the range look larger while the shoulder joint does less. The body stays still, only the forearm moves.

Doing this instead of external rotation is the strategic mistake, and it is common. People see two rotation exercises and assume they should be balanced. They should not, because your programme has already loaded one side heavily and left the other alone. If you only have time for one, it is external rotation.

And programming it as a main exercise wastes session time. Ludewig and Reynolds found scapular kinematics matter for shoulder joint health, and the things that actually improve that picture are pulling volume, external rotation and sensible pressing technique, not a heavier internal rotation.

Sets, reps and rest by goal

Light and brief, when you do it at all.

For general training, two sets of twelve to twenty with forty five seconds rest. If it has been prescribed for a specific reason, three sets of ten to fifteen with a minute, following whatever guidance you were given. In a fat loss phase, two sets of fifteen to twenty five with forty five seconds.

Keep the load low enough that the elbow never moves. That single constraint decides whether the exercise is being done or not.

Progression is not really the point. You are training a pattern with control, not building a muscle you can see, and adding weight tends to produce exactly the compensations that make it useless.

Pallares and colleagues found full range training produced better strength and hypertrophy outcomes than partial work, which here means rotating through the whole comfortable range rather than the middle third.

Schoenfeld and colleagues found a dose response between weekly hard sets and hypertrophy. Applied honestly, that argues for spending those sets on pressing, raises and rowing instead, since the internal rotators already receive plenty of stimulus.

One session a week is more than enough for anyone who is not following a rehabilitation plan.

Where it fits in your split

At the very end of a session, if at all.

On push, pull and legs it goes last on push day. On a bro split it sits at the end of shoulder day. On an upper and lower split it is a brief add-on. On a full body programme it is usually skipped, because the pressing already covers it. On a bodyweight programme push-ups and dips cover it entirely.

The clearer way to think about placement is opportunity cost. Every set you spend here is a set not spent on lateral raises, rows or face pulls, all of which do more for how your shoulders look and feel. If your accessory time is limited, and it usually is, this is the first thing to cut.

The case for keeping it is narrow and specific. Overhead athletes benefit from trained internal rotators in the abducted position, because that is where the demand happens in throwing and serving. And anyone rehabilitating a subscapularis issue has a real reason to be here. Those are good reasons. Symmetry for its own sake is not.

Variations worth knowing

The side lying dumbbell internal rotation is the equipment free version, done lying on your side with a very light dumbbell tucked in.

Band internal rotation anchored at elbow height is convenient, cheap and the format most rehabilitation programmes use.

Internal rotation with the arm abducted to ninety degrees moves the work into the overhead position and is the version that matters for throwing and racquet sports.

Shoulder external rotation is the opposite movement and, for the overwhelming majority of lifters, the one actually worth programming.

The cable chest press is a reminder of how much internal rotation you already do, since it trains the same action alongside the chest and front delt with far more load.

And the face pull is the counterbalancing exercise, covering external rotation, rear delts and mid traps together, which makes it arguably the single most useful shoulder health movement available.

What the evidence says

Kolber and colleagues reviewed shoulder injuries attributed to resistance training and found the shoulder commonly affected among lifters, though the practical implications point towards pulling and external rotation rather than more internal rotation work.

Ludewig and Reynolds found associations between scapular kinematics and glenohumeral joint pathologies, supporting attention to shoulder blade position during any rotation work and explaining why letting the elbow float defeats the exercise.

Sakoma and colleagues described the deltoid as functionally segmented, useful context for understanding that the deltoid does not drive this rotation and that the cuff and the large pulling muscles do.

Saeterbakken and colleagues examined bench press variations and found substantial activity in the muscles that drive the arm inwards and across, which is the evidence behind the claim that your pressing already trains the internal rotators heavily.

Vigotsky and colleagues cautioned against over interpreting raw electromyography amplitudes, which is worth remembering given how much cuff exercise selection is argued on activation numbers alone.

Schoenfeld and colleagues found a dose response between weekly hard sets and hypertrophy, which supports spending limited accessory volume where it produces visible change rather than here.

Pallares and colleagues found full range training beat partial range training, supporting rotating through the whole comfortable arc when you do perform it.

Common questions

Do I actually need this exercise?

Probably not. Your internal rotators are worked heavily by every press, dip and push-up you perform, so they are rarely the weak side of the pair. External rotation is the one that gets neglected and the one worth your accessory time.

When is it genuinely worth doing?

Three situations. If a clinician has identified a subscapularis weakness and prescribed it. If you throw, serve or swim and want internal rotation trained specifically in the overhead position. Or if you have unusually low pressing volume and high pulling volume, which is uncommon but does happen.

Should I balance it one to one with external rotation?

No, and this is the most common error. Balance across a programme means balancing total demand, not exercise count. Your pressing already supplies the internal rotation, so the sensible ratio is heavily weighted towards external rotation or entirely so.

How heavy should it be?

Light enough that the elbow never leaves your ribs and the torso never turns. That is usually the lightest one or two settings on the stack, and adding load beyond that simply recruits the chest.

What should I do instead?

Face pulls, external rotations and rowing volume. Face pulls in particular cover external rotation, the rear delts and the mid traps in one movement, which is why they are the shoulder health exercise most worth defending a slot for.
