How to perform it
Standing side on to a cable set at elbow height with the elbow tucked to your side, rotate the forearm outwards away from your body.
- Set a cable pulley to roughly elbow height and attach a single handle.
- Stand side on with the working arm furthest from the machine.
- Take the handle across your body with the far hand.
- Tuck the working elbow against your ribs and bend it to ninety degrees.
- Place a rolled towel between the elbow and your side if you have one.
- Set the shoulder blade down and back without shrugging.
- Rotate the forearm outwards, away from your stomach.
- Stop when the forearm points forwards or slightly beyond.
- Pause briefly at the end of the range.
- Return slowly across the body under control.

Common mistakes
Elbow drifting away from the ribs
Once the elbow floats, the rear delt and traps take over. Pin it with a towel if needed.
Using far too much weight
The external rotators are small. Heavy loads simply recruit the whole shoulder into a swing.
Turning the torso
Rotating your body rather than your arm makes the range look bigger and trains nothing.
Rushing the reps
Momentum defeats the purpose entirely. Slow and controlled is the only useful tempo.
Treating it as a mass exercise
This is shoulder health and warm-up work. Expecting it to build size leads to loading it wrongly.

What cable shoulder external rotation actually trains
Cable shoulder external rotation is an isolation exercise in which you stand side on to a cable with your elbow tucked against your ribs and rotate your forearm outwards, away from your body.
The muscles doing the work are the external rotators of the rotator cuff, chiefly the infraspinatus and teres minor, with the posterior deltoid contributing a little and the muscles around the shoulder blade working to keep the scapula stable while the arm turns.
Here is the honest framing. This is not a muscle building exercise. The infraspinatus and teres minor are small, they are buried under the deltoid, and no amount of training them will change how your shoulders look. What they do is control the humerus in the socket, and that control is what makes overhead and pressing work feel good rather than pinchy.
So the reason to do it is shoulder health and preparation, not size. A few light sets before pressing, or a couple at the end of the session, is the correct dose and the correct expectation.
That said, of all the rotator cuff work available, external rotation is the one most people genuinely need. Everything you already do in the gym trains the internal rotators. Every press, every dip, every push-up. Almost nothing trains the external rotators, so they are the side of the pair that ends up neglected.
Kolber and colleagues reviewed shoulder complaints attributed to resistance training and found the shoulder to be among the most commonly affected joints in lifters, which is the practical argument for spending five minutes a week here.
The folder name mentions dumbbells, and the side lying dumbbell version is a perfectly good alternative. A cable is better because the resistance is constant through the rotation, whereas a dumbbell loads only part of the arc depending on how you are lying.
Setting up and executing
Set a cable pulley to roughly elbow height and attach a single handle.
Stand side on to the machine with your working arm furthest from it, and take the handle across your body with that hand.
Tuck the working elbow against your ribs, bent to ninety degrees. If it wanders, roll up a towel and hold it there between your elbow and your side; the towel is not a gimmick, it is the simplest way to stop the exercise turning into something else.
Set the shoulder blade down and back without shrugging.
Rotate the forearm outwards, away from your stomach, until it points forwards or slightly beyond. Pause briefly.
Return slowly across the body, keeping the elbow pinned, and repeat.
The mistakes that cost you the most
The elbow drifting away from the ribs is the fault that quietly converts this into a bad rear delt exercise. As soon as the upper arm lifts, the rear deltoid and the traps take the work and the cuff stops being trained. Pin the elbow, use a towel, check it every few reps.
Using far too much weight causes everything else. The external rotators are small muscles and the honest load is embarrassing, often the lightest setting on the stack. If the weight makes you swing, it is too heavy by definition.
Turning the torso makes the range look bigger while actually shortening what the shoulder does. Your body stays still; only the forearm moves.
Rushing the reps defeats the purpose. This is a control exercise. Momentum is the opposite of control.
And treating it as a mass exercise leads people to load it heavily, which produces all of the faults above. Ludewig and Reynolds found scapular kinematics associated with shoulder joint pathology, and the value of this movement is in the quality of the position, not the number on the stack.
Sets, reps and rest by goal
Light, high rep, short rest, and no ego.
For general training, three sets of twelve to twenty with forty five seconds rest. If you are treating it as a strength or rehabilitation piece, three sets of ten to fifteen with a minute. In a fat loss phase, two sets of fifteen to twenty five with forty five seconds, mainly to keep the habit.
The progression here is modest and slow, and that is fine. Adding a couple of kilos to an external rotation over a year is a reasonable outcome; the point was never the load.
Where you place it changes the intent. Two light sets before pressing works as preparation and gets blood into the area. Two or three sets at the end of the session works as accumulated volume for a chronically undertrained muscle. Both are defensible and neither needs to take more than a few minutes.
Schoenfeld and colleagues found a dose response between weekly hard sets and hypertrophy, which is worth mentioning mostly to be clear that it does not really apply here. You are not chasing growth. You are chasing a few hundred controlled repetitions a month in a pattern nothing else in your programme covers.
Two or three sessions a week is plenty.
Where it fits in your split
Before pressing as a warm-up, or after the main work as an accessory. Not in the middle, and never as the focus of a session.
On push, pull and legs it fits either end of push day. On a bro split it goes at the end of shoulder day. On an upper and lower split it works as a short warm-up in either upper session. On a full body programme two light sets before the pressing is enough. On a bodyweight programme a band does the same job.
The honest comparison is with face pulls. A face pull trains external rotation alongside the rear delts and mid traps in one movement, which is why it is arguably the single most useful shoulder health exercise most people can do. If you already do face pulls properly, dedicated external rotation is optional rather than essential.
Where it still earns a place is when you want the rotation isolated, either because a shoulder is grumbling and you want the pattern without the rest of the pull, or because you are warming up and want something specific and quick before you press.
Variations worth knowing
The side lying dumbbell external rotation is the classic version, done lying on your side with a very light dumbbell, and it needs no equipment beyond that.
Band external rotation anchored at elbow height is the most practical warm-up version and travels in a pocket.
Cable external rotation with the arm abducted to ninety degrees trains rotation in a position much closer to where overhead pressing actually happens, and is harder than it looks.
Prone horizontal abduction, face down on a bench raising a light dumbbell out to the side, hits the same general area from a different angle.
The face pull is the compound alternative and covers external rotation, rear delts and mid traps in one movement.
And the Cuban press strings a rotation into a press, which is efficient but demands the rotation is already competent.
What the evidence says
Kolber and colleagues reviewed shoulder injuries attributed to resistance training and found the shoulder among the most commonly affected joints in lifters, which is the core practical argument for including light rotator cuff work.
Ludewig and Reynolds found associations between scapular kinematics and glenohumeral joint pathologies, supporting the emphasis on keeping the shoulder blade set rather than letting it shrug or wing during the rotation.
Sakoma and colleagues described the deltoid as functionally segmented, which is useful context for understanding that the deltoid does not perform this rotation and cannot substitute for the cuff.
Kara and colleagues compared trapezius activation across scapular retraction exercises and found meaningful differences between them, which supports choosing shoulder health exercises deliberately rather than assuming any pulling movement covers the same ground.
Vigotsky and colleagues cautioned against reading too much into raw electromyography amplitudes, which is worth applying to the many claims made about which cuff exercise is best.
Schoenfeld and colleagues on weekly volume applies only loosely here, since the goal is control rather than size.
Pallares and colleagues found full range training produced better outcomes than partial range work, which supports rotating through the whole available range rather than the last few comfortable degrees.
Common questions
How much weight should I use?
Far less than you think. The lightest plate on the stack is a normal starting point, and if the movement makes you swing, lean or lift the elbow, it is too heavy. Nothing about this exercise rewards load.
Is this actually necessary?
Not for everyone. If you already do face pulls or plenty of rowing with good scapular control, you are covering much of the same ground. It becomes worth doing if your shoulders feel rough under pressing, if you press a lot and pull very little, or if you simply want a quick specific warm-up before overhead work.
The folder says dumbbell. Which should I use?
Either works. A cable gives constant resistance through the whole rotation, which is why it is written up here as a cable exercise. The side lying dumbbell version loads only part of the arc but needs no equipment and is perfectly effective, especially at home.
Will this build my shoulders?
No. The external rotators are small and sit under the deltoid, so training them does not change how your shoulders look. Do lateral raises and pressing for that, and do this for the joint.
Should I do internal rotation too?
Rarely. Every press, dip and push-up you do already trains the internal rotators heavily, so they are almost never the neglected side. External rotation is the one worth the five minutes.
