The only exercise that loads the rectus femoris in a lengthened position, and the one still carrying a reputation it never deserved.

How to perform it

Seated in the machine, straighten your knees against the pad, then lower under control without letting the weight rest on the stack.

  1. Set the seat back so the backs of your knees sit right at the edge of the seat.
  2. Adjust the machine so its rotation point lines up roughly with your knee joint.
  3. Set the shin pad so it rests just above your ankles, not on your feet.
  4. Sit down with your back against the pad and your hips fully in the seat.
  5. Hold the handles at your sides so your hips stay down through the set.
  6. Take a breath and brace your abs.
  7. Straighten your knees smoothly against the pad until your legs are fully extended.
  8. Hold the top for a beat without jamming the knees back hard.
  9. Lower under control over about two to three seconds.
  10. Stop just before the plates touch the stack so tension stays on the quads, then repeat.

Common mistakes

Swinging the weight up

Kicking the pad and letting momentum carry the rep, often with the hips bouncing off the seat, removes most of the work. Straighten the knees smoothly and hold the top.

Pad in the wrong place

A shin pad sitting on the feet changes the lever and puts pressure through the ankle. A pad too high up the shin shortens the lever and lets you use weight you cannot control. Just above the ankles is the position.

Resting on the stack between reps

Letting the plates touch down at the bottom of each rep gives the quads a break in the position where tension is most useful. Stop just short.

Dropping the weight down

The lowering half is where the quad works in a lengthened position. Roig and colleagues found eccentric training produced greater strength and mass gains than concentric training, and this is one of the easiest places to take advantage of it.

Too much weight, too short a range

Partial extensions with a heavy stack are common and mostly pointless. Use a load that lets you reach full extension and pause there.

Variations and swaps

Single-leg extension

One leg at a time. Exposes side-to-side differences and lets you give a lagging leg extra work.

Reclined leg extension

Leaning the backrest further back puts the rectus femoris in a longer position at the hip, which loads it harder.

Sissy squat

Knees travelling forward while the hips stay extended. The bodyweight version of a lengthened quad exercise, and demanding on the knees at first.

Reverse Nordic curl

Kneeling and leaning backwards under control. A brutal eccentric quad exercise that needs no equipment.

Bulgarian split squat

A compound quad alternative with a large range at the front knee and a real balance demand.

Hack squat

A heavy compound quad option when you want load rather than isolation.

What the leg extension actually trains

The quadriceps, and only the quadriceps. Four muscles: three vasti that cross only the knee, and the rectus femoris, which crosses the hip as well.

That last one is the reason the leg extension is worth a slot rather than being redundant with squats. The rectus femoris flexes the hip and extends the knee. In a squat, the hip flexes as the knee flexes, so the muscle shortens at one end while lengthening at the other and never gets a good lengthened, loaded position. In a seated leg extension, the hip stays flexed and the knee moves through its full range. Recline the seat further and the hip extends more, lengthening it further still.

So a squat trains the quads well and the rectus femoris poorly. If you want the whole quad, the extension is the cheapest fix available.

Setting up and extending

Seat back so the backs of your knees sit at the edge of the seat, and the machine's pivot lined up roughly with your knee joint. Shin pad just above the ankles, not on the feet.

Sit with your back against the pad and hold the handles so your hips stay down. Brace.

Straighten the knees smoothly until the legs are fully extended, hold for a beat without jamming them back, then lower over two to three seconds. Stop just short of the plates touching so the tension stays on.

That is the whole technique. The only variable worth adjusting deliberately is the backrest angle: more reclined means a longer rectus femoris and a harder exercise at the same weight.

The mistakes that cost you the most

Swinging is the obvious one. Kicking the pad up and letting the hips bounce off the seat means the quad has done a fraction of the work you paid for in time.

Pad position is the one people never check. Sitting on the feet changes the lever and puts pressure through the ankle; too high up the shin lets you load weight you cannot actually control.

Resting on the stack between reps is a small leak that adds up, since the bottom of the range is where the tension matters most.

And the lowering. Roig and colleagues found eccentric training produced greater strength and hypertrophy gains than concentric in their meta-analysis, and Schoenfeld and colleagues found a small hypertrophy advantage for eccentric-focused work. On a machine with a fixed path and no balance demand, taking three seconds to come down costs you nothing and is free work.

How to programme the leg extension

Three to four sets of ten to fifteen with 90 seconds of rest. This is an isolation exercise on a machine, so higher reps and short rest suit it and there is nothing to gain from very heavy triples.

It is one of the best places in the whole gym to train to failure. Nothing to drop, no spotter, no balance. Refalo and colleagues found the difference from training closer to failure was modest once volume was equated, so it is a choice rather than an obligation, but if any set in your leg session ends at the end, this is the one.

For weekly volume, extensions are a cheap way to add quad sets that cost the lower back nothing. Schoenfeld and colleagues found the dose-response relationship between weekly sets and hypertrophy, and after a heavy squat day the extension machine lets you keep adding quad work when nothing else can.

Drop sets and long final sets work well here for the same reason: the failure point is safe.

Where the leg extension sits in your week

Late in a leg session, after the squats and presses. On push/pull/legs, on an upper/lower split, on a bro split, the answer is the same: it is an accessory, and it goes where accessories go.

The one situation where it moves forward is as a warm-up. Two light sets before squatting warms the knees and quads and costs nothing.

If your quads are a priority, adding a couple of sets of extensions to the end of any lower body session is the least fatiguing way to raise weekly quad volume.

Variations and swaps

Single-leg extensions expose the strength difference between your legs, which two-legged work hides completely.

Reclining the backrest is the most useful adjustment: more hip extension means a longer rectus femoris and a harder set at the same load.

Without a machine, sissy squats and reverse Nordic curls load the quad in a lengthened position using body weight alone. The reverse Nordic in particular is a genuinely hard eccentric quad exercise and needs nothing but a pad for your knees. Both need building up to slowly.

What the evidence does and does not say

The persistent claim is that leg extensions are bad for your knees, because open-chain knee extension produces high shear force at the joint. Escamilla and colleagues measured knee forces across knee-extending exercises and found the patterns differ between open and closed chain movements, with the highest stress on the extension occurring near full extension rather than throughout.

What has not been shown is that healthy people who do leg extensions get injured knees. The concern comes from measurements in a lab and from rehabilitation contexts where a specific ligament is being protected. If you have a diagnosed knee problem, particularly a cruciate ligament, take the advice of whoever is treating it. If you do not, the leg extension is an unremarkable isolation exercise, and there is a reasonable case that regularly loading the quads through a full range makes a knee more robust rather than less.

It also does not say the leg extension is a substitute for squatting. It is an addition, and its specific value is the rectus femoris.

Leg extension: common questions

Are leg extensions bad for your knees?

Not for healthy knees. The concern comes from laboratory measurements of joint force and from rehabilitation settings where a specific ligament is being protected. If you have a diagnosed knee problem, follow the advice of whoever is treating it; otherwise the exercise is unremarkable.

Why do leg extensions if I already squat?

Because the rectus femoris crosses the hip as well as the knee, and squatting never puts it in a loaded lengthened position. Extensions, particularly with the backrest reclined, do exactly that.

Where should the shin pad sit?

Just above your ankles. On your feet it changes the lever and presses on the ankle joint. Too high up the shin and you can load more weight than you can genuinely control.

Should I go to failure on leg extensions?

It is one of the safest places in the gym to do so, since there is nothing to drop and no spotter needed. The benefit over stopping a rep or two short is modest, but if any set in your session ends at failure, this is a sensible one.

How fast should I lower the weight?

Two to three seconds. The lowering half is where the quad works in a lengthened position, and on a machine with a fixed path there is no reason to hurry it.
