How to perform it

From a chin down position, extend your head backwards against light resistance, then lower under control.

  1. Lie face down on a bench with your head and neck clear of the end.
  2. Place a folded towel on the back of your head.
  3. Rest a light plate on the towel and hold it there with one hand.
  4. Let the head hang so the chin approaches the chest.
  5. Brace the torso and press the chest into the bench.
  6. Extend the head backwards smoothly over about three seconds.
  7. Stop when the head reaches roughly neutral with the spine.
  8. Hold there for a second without forcing further.
  9. Lower over about three seconds back towards the chest.
  10. End the set as soon as the movement stops being smooth.

Common mistakes

Too much load, too soon

Neck muscles are small and the joint is unforgiving of enthusiasm. Start with no weight and progress over months rather than weeks.

Extending as far as possible

Stopping around neutral is enough. Cranking the head backwards adds no useful work and is where discomfort usually starts.

Using the upper back

If the chest lifts off the bench, the spinal erectors are doing the lifting instead of the neck.

Fast or jerky reps

Momentum on the cervical spine is not worth it for any amount of extra load. Slow both directions.

Skipping the other directions

Extension alone builds the back of the neck and leaves flexion and side flexion untrained, which is a half finished job.

What the neck extension actually trains

A neck extension is an isolation exercise in which you start with the chin towards the chest and extend the head backwards against light resistance, usually lying face down on a bench with a plate on the back of the head.

The splenius capitis, the semispinalis capitis and cervicis, the upper trapezius and the levator scapulae share the work. They run from the base of the skull down into the upper back and they are what gives a trained neck its thickness from behind and from the side.

Neck training is legitimate, and the case for it is easy to make honestly. For anyone in a collision or combat sport, a stronger neck is one of the few genuinely sport specific adaptations you can pursue in a gym. For anyone training for how they look, the neck sits next to your face in every photograph.

The case has limits. This is not a posture cure. Posture is a matter of the positions you occupy for hours at a time. It is also not a headache treatment, whatever the internet says. Train the neck to have a bigger, stronger neck.

Finally, the load warning this muscle group deserves. Progress on the neck slowly. This is the one place where "start lighter than you think" is a genuine recommendation rather than a disclaimer, because the muscles are small, the range is short and the cervical spine is not somewhere to discover your limits.

Setting up and executing

Lie face down on a flat bench with your head and neck clear of the end, so your head can hang freely.

Place a folded towel across the back of your head, rest a light plate on the towel and hold it there with one hand. If you have no plate, use your hands alone as the resistance.

Let your head hang so the chin approaches the chest. That hanging position is the start of every rep.

Brace the torso and press your chest down into the bench. Nothing below the neck should move.

Extend the head backwards smoothly over about three seconds, stopping when your head reaches roughly neutral with your spine. Hold for a second without pushing further.

Lower over about three seconds back towards the chest.

End the set as soon as the movement stops being smooth. Technique failure is the only failure worth using on the neck.

The mistakes that cost you the most

Too much load, too soon is the mistake that defines neck training. People treat a five kilo plate as trivial because it is trivial everywhere else, then spend three days unable to look over their shoulder in the car. Begin with no added weight, use your hands for the first fortnight, then add the smallest plate you can find.

Extending as far as possible is the range error. The productive range ends around neutral. Cranking further adds no useful work and takes you into the position where discomfort usually starts.

Using the upper back is the compensation. If your chest is lifting off the bench, the spinal erectors have taken over and your neck is going along for the ride.

Fast or jerky reps trade a little more load for momentum applied to the top of your spine, which is a poor deal at any weight.

And skipping the other directions leaves the job half done. Flexion and lateral flexion cover the rest, and a neck trained in one direction only is an odd thing to aim for.

Sets, reps and rest by goal

Moderate to high reps, slow tempo, light load, and a long time horizon.

For muscle gain, three sets of twelve to twenty with ninety seconds of rest. For strength, three sets of ten to fifteen with two minutes. In a fat loss phase, two sets of fifteen to twenty with a minute.

Progress by adding reps for several weeks before adding any weight, then add the smallest increment available and expect the reps to fall back. Moving up by a kilo or two over a month is respectable here, and rushing it is the most common way people end up abandoning neck training altogether.

Schoenfeld and colleagues found a dose response between weekly hard sets and hypertrophy, measured in conventional resistance exercises rather than in neck work. The general principle carries: a couple of hard sets twice a week accumulate into something, and one occasional set does not.

Refalo and colleagues found training a rep or two short of failure produced growth comparable to training to failure in loaded resistance exercise. That is convenient here, because true failure on a neck exercise is not something to seek out.

Schoenfeld and colleagues, in a separate review, found eccentric and concentric actions produce broadly similar hypertrophy, which means the slow lowering here is about control and joint comfort rather than a superior growth stimulus.

Where it fits in your split

At the end of a session, once or twice a week.

On push, pull and legs it goes at the end of pull day. On a bro split it closes back day. On an upper and lower split, one upper session a week is enough. On a full body programme it is worth keeping only if a contact sport or neck size is a stated priority.

Always after the heavy work, never before. A tired neck makes bracing under a bar unpleasant.

Frequency deserves a note of its own. The neck is working all day, holding a head that weighs several kilos in whatever position you have put it in. That background load means it recovers more slowly than a muscle group this size would imply. Two sessions a week is plenty, and starting with one is sensible.

Pair it with flexion and lateral flexion work in the same block. All three directions, two or three sets each, takes about ten minutes. Kara and colleagues found the portions of the trapezius contribute differently depending on the movement, which illustrates why one direction does not cover a region with several jobs.

Variations worth knowing

The cervical neck extension is the seated or standing version, using a head harness or manual resistance. The muscles and the range are the same, and the choice is mostly about equipment.

Manual resistance extension, with your own hands supplying the load, is self limiting by design and is the right way to spend your first few weeks.

Band neck extension, with a band anchored in front of you, loads the shortened end of the range more than a plate does.

Neck flexion trains the front, chiefly the sternocleidomastoid and the deep neck flexors, and belongs alongside this exercise rather than instead of it.

Lateral neck flexion, taking the ear towards the shoulder, covers the sides.

And the wrestler's bridge is the traditional heavy option, effective for people who have been doing this for years and unsuitable for anyone who has not.

What the evidence says

There is very little research on neck training specifically, so what follows describes what these studies measured in conventional resistance exercise and applies the principles openly rather than pretending they were tested on the neck.

Pallarés and colleagues found full range training produced better outcomes than partial range work, which supports using your available pain free range rather than a token movement.

Pedrosa and colleagues found training at long muscle lengths produced favourable hypertrophy outcomes, which is the argument for starting each rep from a genuine chin towards chest position.

Schoenfeld and colleagues found a dose response between weekly hard sets and muscle growth, which sets the expectation that visible neck development comes from accumulated volume over months.

Refalo and colleagues found training close to but short of failure produced comparable hypertrophy to training to failure, which is a useful licence to keep neck sets conservative.

Schoenfeld and colleagues, in a review of concentric and eccentric actions, found broadly similar hypertrophy between them, so the slow lowering here is chosen for control rather than for a growth advantage.

McGill wrote on core training and how stiffness and control translate to performance and injury prevention, the nearest principle in the pool to bracing the torso so the neck works in isolation.

Kara and colleagues measured trapezius activation during scapular retraction exercises and found its portions contribute differently by task, supporting the point that neck and upper back work are not substitutes.

Common questions

How is this different from the cervical neck extension?

There is barely a difference. Both extend the head backwards against resistance and train the same muscles. This version is usually done lying face down with a plate; the other is usually seated or standing with a harness. Choose one and do not do both.

Do shrugs train the neck?

No. Shrugs train the upper trapezius, which contributes to how thick your neck looks from behind but is not the neck itself. If you want the splenius and semispinalis to grow, they have to be loaded through extension specifically.

How quickly can I add weight?

Far more slowly than on any other exercise. Add reps for several weeks, then add the smallest plate available, then hold that for several more weeks. A kilo or two a month is good progress, and treating it like a bench press is how people end up with a sore neck and no training habit.

Is neck training safe?

It is, provided the load is light, the tempo is slow and you stay inside a comfortable range. The risk comes entirely from enthusiasm: heavy plates, fast reps and forcing the end position.

Should everyone train their neck?

No. It is worthwhile for collision and combat sport athletes, and reasonable for anyone who wants a thicker neck. Outside those two cases it is optional.
