How to perform it
Seated or standing with a head harness or your hands behind your head, extend your neck backwards against the resistance, then return under control.
- Sit on a bench with your feet flat and your torso upright.
- Fit a head harness snugly, or place both palms on the back of your head.
- If using a harness, hang a very light plate from the chain.
- Brace the torso so only the neck will move.
- Lower the chin towards the chest until you feel a mild stretch.
- Extend the neck backwards smoothly over about three seconds.
- Stop when the head is roughly upright to slightly past neutral.
- Hold the end position for a second without straining.
- Return to the chin down position over about three seconds.
- Keep the shoulders down and still throughout the set.

Common mistakes
Starting too heavy
This is the one muscle group where starting lighter than you think is a genuine instruction rather than filler. Begin with no added load at all.
Moving the torso instead of the neck
Rocking backwards from the hips or upper back turns it into a back extension. Only the head should travel.
Rushing the reps
Fast neck work is momentum plus a small joint. Three seconds each direction is the minimum.
Forcing range at the top
Cranking the head as far back as it will go adds nothing useful and is the position people most often aggravate.
Expecting it to fix posture or headaches
Neck training builds neck muscle. It is not a posture correction programme and it is not a headache treatment.

What the cervical neck extension actually trains
A cervical neck extension is an isolation exercise in which you extend your head and neck backwards against resistance from a head harness, a band or your own hands, usually seated or standing.
The splenius capitis, the semispinalis capitis and cervicis, the upper trapezius and the levator scapulae share the work. Together they run from the base of the skull down into the upper back, and they are the muscles that hold your head up all day and resist it being knocked around.
Neck training is legitimate and undertrained. If you play rugby, wrestle, box or do any grappling sport, direct neck work is one of the few genuinely sport specific things you can do in a gym. If you simply want the look, a thicker neck changes a physique more than almost any other small muscle group, because it sits next to your face in every photograph.
Two things it is not. It is not a posture cure. Posture is largely a matter of position, habit and time spent in one shape. It is also not a headache treatment. Neck training builds neck muscle, and claims beyond that belong to clinicians rather than coaches.
The other honest note is about load. This is the one muscle group where "start lighter than you think" is a real instruction rather than a hedge. The joint is small, the range is short, and the consequences of an aggravated cervical spine are out of proportion to the size of the exercise.
Setting up and executing
Sit on a bench with your feet flat and your torso upright. If you are using a head harness, fit it snugly so it cannot slide, and hang a very light plate from the chain. Otherwise place both palms on the back of your head.
Brace the torso so that only the neck can move. This is the detail that separates neck training from rocking backwards and forwards.
Lower your chin towards your chest until you feel a mild stretch through the back of the neck.
Extend the neck backwards smoothly over about three seconds, until your head is roughly upright or a little past neutral. Hold for a second without straining.
Return to the chin down position over about three seconds, resisting the whole way.
Keep the shoulders down and still. If they rise towards your ears, the traps have taken over and the load is too heavy.
Stop the set where the movement stops being smooth. On the neck, technique failure is the only failure worth using.
The mistakes that cost you the most
Starting too heavy is the mistake that matters most. People bring their normal training instincts to the neck, load a plate on a harness because a plate feels small, and discover a day later that a small joint under an unfamiliar load complains loudly. Start with your own hands or an empty harness for the first two or three weeks.
Moving the torso instead of the neck is the compensation that follows heavy loading. If your chest is travelling backwards and your ribs are flaring, you are doing a seated back extension with a harness attached.
Rushing the reps is a tempo problem with real consequences. Momentum on a small joint at the top of your spine is not something to invite. Three seconds in each direction is the floor.
Forcing range at the top adds nothing. The useful range ends around neutral to slightly beyond, and cranking the head as far back as the joint permits is where people most often feel it the next day.
And expecting it to fix posture or headaches is an expectation error rather than a technique one. Train the neck to have a stronger, bigger neck.
Sets, reps and rest by goal
Moderate to high reps, slow tempo, conservative load, and a long runway.
For muscle gain, three sets of twelve to twenty with ninety seconds of rest. For strength, three sets of ten to fifteen with two minutes. In a fat loss phase, two sets of fifteen to twenty with a minute.
Progress on the neck slowly, and mean it. Add reps for two or three weeks at a given load, then add the smallest plate available, then stay there for another few weeks. Adding a kilo a month to neck work is fast progress, not slow.
Schoenfeld and colleagues found a dose response between weekly hard sets and hypertrophy, which they measured in conventional resistance exercises rather than in neck training. The general principle still applies: a few hard sets twice a week will do more than one occasional set.
Refalo and colleagues found stopping a rep or two short of failure produced comparable growth to going to failure, again in conventional lifts. On the neck that is unusually convenient, because training to true failure here is a bad idea.
Roig and colleagues found eccentric training produced greater strength gains than concentric training, which supports the deliberate three second return rather than letting the head drop.
Where it fits in your split
At the end of a session, twice a week.
On push, pull and legs it goes at the end of pull day, after the back work. On a bro split it closes back or shoulder day. On an upper and lower split it can be added to either upper session. On a full body programme it is worth protecting if you play a collision or combat sport, and easy to drop otherwise.
Do it after your heavy pulling rather than before. A fatigued neck makes bracing under a bar less pleasant.
Two sessions a week is the sweet spot. The neck recovers more slowly than people assume from a muscle group this size, largely because it is working all day whether you train it or not.
Train the other directions too. Extension alone leaves flexion and lateral flexion untouched, which is a half job. Kara and colleagues examined trapezius activation during scapular retraction work and found meaningful differences in how the trapezius portions contribute, a reminder that this is a region with several jobs rather than one muscle.
Variations worth knowing
Neck extension performed lying prone, with a plate on the back of the head, is the same movement with easier and more precise loading, and is probably the version most people should default to.
Manual resistance extension, using your own hands to supply the load, is self limiting by design, which makes it the safest starting point.
Neck flexion is the opposite movement, training the sternocleidomastoid and the deep neck flexors, and it belongs alongside this one.
Lateral neck flexion, taking the ear towards the shoulder, covers the sides.
Isometric neck holds, pressing into a fixed hand or pad without moving, are the gentlest entry point and a sensible way to spend your first fortnight.
And shrugs, while not neck training at all, build the visible upper trapezius that people often mean when they say they want a thicker neck.
What the evidence says
The evidence pool for neck training specifically is thin, so what follows describes what these studies measured in conventional resistance training and applies the principles openly rather than pretending anyone tested them on the neck.
Pallarés and colleagues found full range training produced better outcomes than partial range work in resistance exercises, which supports using your available pain free range rather than a token nod of the head.
Schoenfeld and Grgic reviewed range of motion and muscle development and reached a similar conclusion, with fuller ranges generally favoured.
Schoenfeld and colleagues found a dose response between weekly hard sets and muscle growth, which sets the expectation that neck size responds to accumulated volume like anything else.
Refalo and colleagues found training close to but short of failure produced comparable hypertrophy to training to failure, a useful licence to keep neck sets conservative.
Roig and colleagues found eccentric training produced greater strength gains than concentric training, supporting the slow controlled return.
McGill wrote on core training and the translation of stiffness and control to performance and injury prevention, the closest principle in the pool to bracing a stable torso before training a small joint.
Kara and colleagues measured trapezius activation during scapular retraction exercises and found its portions contribute differently by movement, which is why upper back and neck work are not interchangeable.
Common questions
How is this different from the neck extension entry?
Barely at all, and it is worth saying so plainly. Both train the posterior neck through extension. This version is typically done seated or standing with a harness or your hands; the other is more often done lying prone with a plate. Pick whichever your equipment suits.
How much weight should I start with?
None. Use your own hands or an empty harness for the first two or three weeks, then add the smallest plate available.
Will neck training improve my posture?
No, not in the way it is usually sold. Posture is mostly about position and habit over hours, not about the strength of one muscle group.
Should I train the neck if I do not play a contact sport?
Only if you want to. There is a legitimate case for combat and collision athletes, and an aesthetic case for anyone who wants a thicker neck. Outside those two reasons it is optional.
How often can I train my neck?
Twice a week is enough. The neck works all day holding your head up, so it accumulates more background load than its size suggests, and stacking frequent hard sets on top of that is how people end up sore in a place that makes sleeping unpleasant.
